Stephen P. Coltharp, MD, FACEP is a board-certified emergency medicine physician and healthcare leader with more than 15 years of experience delivering high-quality acute and emergency care. He currently practices emergency medicine in Florida and serves as Assistant Medical Director for Flagler County’s prehospital emergency medical service organizations. He is committed to expanding timely, patient-centered medical care beyond traditional clinical settings—leveraging technology to deliver safe, efficient, and accessible healthcare when and where patients need it most.

Emergency Telemedicine Nurse Practitioner
Barbara A. Thomason is a co-founder of Code3MD and a board-certified Emergency Nurse Practitioner with over 27 years of experience spanning the full continuum of patient care.
Her career began in the field as a Firefighter/Paramedic working at the front line of patient stabilization. She went on to serve as an Emergency Department, Trauma, ICU, and Critical Care Registered Nurse, later advancing into her role as a Nurse Practitioner specializing in emergency and urgent care medicine.
Her vision is rooted in real-world experience: understanding where patients fall through the cracks and creating solutions that are both clinically sound and operationally effective. Code3MD reflects that mission—delivering accessible, efficient, and expert-level care to communities locally and beyond.
In addition to her clinical work, Barbara has served as a nursing educator, medical consultant, and small business owner. She is recognized for her leadership, clinical acumen, and unwavering commitment to elevating patient care standards across every setting she serves.
EMS will identify patients that may benefit from meeting with a physician on site in lieu of being transported to the emergency department for care.
The cost for a telemedicine visit through Code3MD is comparable to a standard office visit. Specific fees may vary depending on the nature of the consultation and your insurance coverage.
Most major insurance plans are accepted for telemedicine services provided by Code3MD. Coverage may vary, so we recommend contacting your insurance provider for details regarding your specific plan.
We will always have a physician available, but we also work with physician’s assistants and nurse practitioners with EMS and emergency department experience.
We can arrange a follow up telemedicine visit, or guide you through an in-person follow up visit with a primary care physician or specialist.
Yes. The decision to report to the emergency department will always remain with the patient.

We are a company created to fill a need. Historically, more than 30% of patients that present to emergency departments nationwide neither need or want to be there. They present as a result of system failures that left them no other option. We created Code3MD to be another option!
Florida is the fastest growing state in the nation. We have added around 2 million people over the past 5 years, and all of those people need face time with physicians. Our expertise is emergency medicine, but we frequently are the bridge to connect patients with other specialties or social support systems.
Code3MD is activated when paramedics meet with a patient they believe could have their needs met from the comfort of their own home, thereby eliminating the need for sometimes risky and always costly transport. They call us to give us relevant information, and then connect us to patients via video chat or telephone on the patient’s own technological device. One is provided if the patient does not own a device.
A Code3MD provider will finish the encounter remotely, including patient instructions, prescriptions, and/or referrals. If at any time the provider or the patient believe they will be better served in the emergency department, EMS transport is activated.
We also operate by elective appointment; patients can book follow up and initial encounters online.
